Job Summary:

Job Summary:

Job Summary:

Under the direction of the Quality Improvement Director, the primary function of the Patient Engagement Coordinator is to work as part of a patient care team to provide high quality, efficient and patient focused support while demonstrating the health center’s core values. Working in coordination with physicians, mid-level providers, nurses, medical assistants and care coordinators, the Patient Engagement Coordinator assists patients in the coordination and proactive facilitation of access to patient care services. The Patient Engagement Coordinator is responsible for management and coordination of care for assigned population health lists and uninsured populations. Employing population management techniques, the Patient Engagement Coordinator works with existing and new assigned patients to coordinate health care and enabling services. The Patient Engagement Coordinator is responsible for proactively working with the patient care team to improve coordination of care for patients and facilitate access to primary care services as well as specialty care appointments and hospital follow up care. The Patient Engagement Coordinator works closely with the care teams to organize activities necessary to successfully document and report back to regulatory entities and payers to meet requirements established by managed care plans (e.g. HEDIS clinical quality measures, FQHC clinical performance measures, and Initial Health Assessments).

The PSR Lead is responsible for effectively communicating with patients for the purpose of obtaining and relaying accurate information needed for processing their medical visit. The PSR Lead’s job responsibilities include coordinating and facilitating the daily workflow of the PSR team as well as fulfilling all of the duties assigned to the PSR position. The PSR Lead is responsible for handling customary day-to-day operational challenges and for working with their team and other health center personnel to solve routine problems. The PSR Lead models the health center’s core values and actively works with their teams to deliver a high level of customer service. The PSR Lead is responsible for actively participating in the health center’s quality and continuous improvement activities.

Job Summary:

The Associate Behavioral Health Specialist will work under the direct supervision and training of the Behavioral Health Director. The Associate Behavioral Health Specialist provides behavioral health services, including conducting complete brief and focused assessments utilizing the biopsychosocial approach, using evidence-based brief therapy, being flexible, and practicing in a fast-paced clinic environment. The Associate Behavioral Health Specialist work alongside the health center’s medical teams to provide comprehensive whole-person care. They will offer same-day mental health consults, psychosocial assessments, crisis intervention, psychotherapy, alcohol, and drug screening using the SBIRT (Screening, Brief Intervention, and Referral to Treatment), education/support groups, and resource information. The Associate Behavioral Health Specialist is the bridge between Camino Health Center, the community, and other local agencies that provide this population’s services.

Minimum Position Qualifications:

  • Education: Master’s degree in Social Work from a program accredited by the Commission on Accreditation of the Cultural Humility UDS Chronic Management Council on Social Work Education (CSWE).
  • License / Certification: Valid Associate Clinical Social Worker (ACSW) license issued by the California Board of Behavioral Health Sciences is required. It must be maintained to continue recording experience hours until licensed as an LCSW—current BLS certification.
  • Experience / Training: 1-2 years of experience providing direct behavioral health services to patient of all ages; ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions; should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong communication skills

Job Summary:

Job Summary:

Job Summary:

Responsible for the delivery of patient-centered, team-based care that falls within the allowable scope of practice. This includes working collaboratively with physicians, registered nurses and medical assistants to provide advanced practice physical and psychosocial assessments, diagnosis and management of health and illness needs. The primary responsibility of the Nurse Practitioner or Physician Assistant is direct patient care for all ages and genders, including both acute and chronic care.
