Job Summary:
The Special Supplemental Nutrition Program for Women, Infants and Children (WIC) provides nutrition education, referrals and nutritious foods to eligible pregnant, postpartum, and lactating women and their children under the age of 5. Under the direction of the WIC Supervising Nutritionist, is responsible for WIC participant services and the efficient flow of WIC participants through the clinic. Provide accurate appointment scheduling, intake, and processing of participants. Screen and document applicants’ information for WIC Program eligibility, and issue benefits when appropriate. Greet and promote positive working relationships with clients, families, health care providers, and co-workers. Perform other duties as assigned.

Job Summary:
Under the direction of the Clinic Manager, the Case Manager works as part of the Enhanced Care Management (ECM) team and Primary Care Provider (PCP) care team to provide high quality, efficient and service oriented patient/member care while demonstrating the health center’s core values. The Case Manager is responsible for all phases of case management. The Case Manager uses an integrated (medical/behavioral health) case management program and a collaborative relationship-based approach, to assist poly-chronic and high-risk patients to improve health outcomes. Case Managers use a comprehensive, whole person health assessment of the patient’s current situation and status, identifies actionable barriers and obtainable goals to achieve optimal self-care management and monitors the patient’s progress towards their goals. Utilizing Camino’s electronic health record system, the Case Manager is responsible for capturing, monitoring and working all case management referrals requested by the providers. The Case Manager works closely with ECM and their care team to organize activities necessary to successfully meet requirements established by managed care plans, HEDIS clinical quality measures and FQHC clinical performance measures.

Job Summary:
Responsible for the delivery of patient-centered, team-based care that falls within the allowable scope of practice. This includes working collaboratively with physicians, registered nurses, and medical assistants to provide advanced practice physical and psychosocial assessments, diagnosis, and management of health and illness needs. The primary responsibility of the Nurse Practitioner is direct patient care for all ages and genders, including both acute and chronic care. The Nurse Practitioner will also assist the Chief Medical Officer and Staff Physicians in supervision of the nursing staff.

Job Summary:
The Registered Dental Assistant’s (RDA) primary job function is to work as part of a team to provide high quality, efficient and service oriented patient care while demonstrating the health center’s core values. The RDA is responsible for performing basic supportive chair-side procedures under the direction of the Staff Dentists, maintaining the operatories in a safe and workable condition, properly cleaning all dental instruments, and assistance with providing oral health education to patients and caregivers. The registered dental assistant is charged with providing positive, supportive and caring behavior in all communications to patients and families. The RDA is responsible for mentoring and supporting Dental Assistants in the essential functions associated with Direct Patient Care and Back Office Duties.

Under the direct supervision of the Clinic Manager, the Patient Services Representative (PSR) Lead is responsible for providing a high level of customer service to Camino’s patients, community partners and other health center employees. The PSR Lead is responsible for effectively communicating with patients for the purpose of obtaining and relaying accurate information needed for processing their medical visit. The PSR Lead’s job responsibilities include coordinating and facilitating the daily workflow of the PSR team as well as fulfilling all of the duties assigned to the PSR position. The PSR Lead is responsible for handling customary day-to-day operational challenges and for working with their team and other health center personnel to solve routine problems. The PSR Lead models the health center’s core values and actively works with their teams to deliver a high level of customer service. The PSR Lead is responsible for actively participating in the health center’s quality and continuous improvement activities.
Job Summary:
The Associate Behavioral Health Specialist will work under the direct supervision and training of the Behavioral Health Director. The Associate Behavioral Health Specialist provides behavioral health services, including conducting complete brief and focused assessments utilizing the biopsychosocial approach, using evidence-based brief therapy, being flexible, and practicing in a fast-paced clinic environment. The Associate Behavioral Health Specialist work alongside the health center’s medical teams to provide comprehensive whole-person care. They will offer same-day mental health consults, psychosocial assessments, crisis intervention, psychotherapy, alcohol, and drug screening using the SBIRT (Screening, Brief Intervention, and Referral to Treatment), education/support groups, and resource information. The Associate Behavioral Health Specialist is the bridge between Camino Health Center, the community, and other local agencies that provide this population’s services.

Job Summary:
Under the direct supervision of the Development Director, the Development Associate will support the planning, organizing, and the implementation of fundraising including major gifts, planned giving, private grants, special events, capital campaigns, and Camino Health Center Auxiliary. The Development Associate will be responsible for maintaining the organization’s donor database using Blackbaud’s Raiser’s Edge NXT.

Under the direct supervision of the Clinic Manager, the Health Information Clerk is responsible for providing a high level of customer service to Camino’s patients, community partners, and other health center employees. The Health Information Clerk is responsible for effectively processing, organizing, and distributing medical records in accordance with health center policies and applicable HIPAA regulations. Responsibilities include monitoring electronic fax queues, indexing patient records, acting on medical records queues, facilitating the release of medical records, and answering medical phone calls.

Job Summary:
Direct Patient Care: Provide high quality, patient-centered and values-based medical care, referral of patients for specialty care as needed, collaboration with health care team members as appropriate for consultation, referrals and follow-up care (80%).
Supervision of Clinical Staff: Assist mid-level health care providers in assessment and treatment of patients as necessary and direct supervision of nursing staff (10%).
Patient Education: Direct patient education regarding medical conditions and treatment, appropriate verbal and written patient referrals for further health education, assist CEO and CMO in program development and implementation as necessary (5%).

The medical assistant is responsible for executing standing orders as well as anticipating the needs of the care team in order to help facilitate the delivery of quality clinical care and a positive patient experience. The medical assistant is responsible for actively participating in the health center’s quality and continuous improvement activities.

Job Summary:
Under the supervision of the Site Director, the Clinic Manager is responsible for coordinating the efficient flow of medical, behavioral health and dental services, ensuring a high level of customer service and managing the health center’s telecommunications and electronic health records systems. The Clinic Manager is responsible for organizing and coordinating the day-to-day activities of the medical and dental support staff. The Clinic Manager is charged with maximizing appointment scheduling, ensuring patients are check-in accurately, and facilitating proper registration for public and privately funded programs. Participates and leads process improvement initiatives to enhance the health center’s Patient Centered Medical Home (PCMH) care delivery model. The Clinic Manager is responsible for promoting positive working relationships with patients, families, health care providers and health center employees in the spirit of Camino’s core values. The Clinic Manager is charged with developing a culture of teamwork within the health center in order to ensure an excellent patient experience, efficient operations and optimal access to care.
The Clinic Manager directly supervises the Health Information Clerks, Patient Services Representatives, Patient Services Representative Leads, Medical Assistants, Primary Care Team Leads, Perinatal Coordinator, Dental Assistants, Registered Dental Assistants, and Dental Leads.
The Clinic Manager is an active member of the Management Team.

The Behavioral Health Specialist is the link between Camino Health Center, the community, and other local agencies that provide services for this population. Behavioral health services will be provided on site at Camino Health Center and with other community partnerships.
