Job Summary:

Under the direct supervision of the Development Director, the Development Coordinator will support the planning, organizing, and the implementation of fundraising including major gifts, planned giving, private grants, special events, capital campaigns and Camino Health Center Auxiliary. The Development Coordinator will be responsible for maintaining the organization’s donor database using Blackbaud’s Raiser’s Edge NXT.

Minimum Position Qualifications:

  • Education: High School diploma or equivalent.
  • Experience / Training: One year working experience in fundraising for a nonprofit healthcare or social service organization. Proficient in Microsoft office applications.

Preferred Position Qualifications:

  • Education: Bachelor’s degree in Communications, Marketing, or related field from an accredited university.
  • Experience / Training: Raisers Edge NXT, familiarity with design programs a plus.

Job Summary:

Under the general supervision of the Site Director, the Nursing Manager is accountable for planning, organizing, coordinating and managing the operations of the Care Coordination Department and Laboratory. Assures that quality care is provided in the most effective and efficient manner, coordinating service delivery and relationships with community resources and community partners. Adheres to state and federal guidelines relating to primary care and laboratory services. Formulates, recommends, evaluates and interprets clinical policy in accordance with standards. Works collaboratively with the Chief Medical Officer and Quality Improvement Manager to support the efforts to ensure delivery of effective patient care services while promoting a safe environment. Provides supervision to the Care Coordinators and Lab Technician.

The Nursing Manager is an active member of the Management Team.

Job Summary:

The Medical Director is responsible for leadership and oversight of the medical services programs including the provision of direct care. Provides leadership in clinical and administrative meetings as appropriate and works collaboratively with the Site Director to ensure high quality care for patients. The Medical Director mentors and supervises the medical providers and provides support to other programs and departments as needed. The Medical Director collaborates with the management team to address clinical, programmatic and administrative issues.

The Medical Director is an active member of the Camino Health Center Management Team.

Minimum Position Qualifications:

  • Education: Doctor of Medicine Degree or Doctor of Osteopathic Medicine Degree
  • Experience / Training: A minimum of two years of experience in a primary care setting. Two years supervisory experience of other clinical staff.
  • License / Certification: Board certified primary care physician (Family Practice or Internal Medicine); valid and current California Medical license, DEA number and BLS certification.

Preferred Position Qualifications:

  • Education: Master’s in Public Health or Administration or related field
  • Experience / Training: Prior experience working for a Federally Qualified Health Center or community health center; bilingual (English/Spanish) and/or (English/Farsi)

Job Summary:

Direct Patient Care: Provide high quality, patient-centered and values-based medical care, referral of patients for specialty care as needed, collaboration with health care team members as appropriate for consultation, referrals and follow-up care (80%).

Supervision of Clinical Staff: Assist mid-level health care providers in assessment and treatment of patients as necessary and direct supervision of nursing staff (10%).

Patient Education: Direct patient education regarding medical conditions and treatment, appropriate verbal and written patient referrals for further health education, assist CEO and CMO in program development and implementation as necessary (5%).

Quality Management and Utilization Review: Practice in an efficient, cost effective manner, participation in department and clinic-wide meetings, attend relevant Continuing Medical Education courses (5%).

Job Summary:

The medical assistant’s primary job function is to work as part of the primary care team to provide high quality, efficient and service oriented patient care while demonstrating the health center’s core values. The medical assistant works alongside physicians, mid-level providers, behavioral health professionals, registered nurses and other support staff support to delivery direct patient care services. The medical assistant is responsible for executing standing orders as well as anticipating the needs of the care team in order to help facilitate the delivery of quality clinical care and a positive patient experience. The medical assistant is responsible for actively participating in the health center’s quality and continuous improvement activities.

Job Summary:

The Behavioral Health Specialist provides behavioral health services to patients of all ages at Camino Health Center. The Behavioral Health Specialist work alongside the health center’s medical teams to provide comprehensive whole person care. The Behavioral Health Specialists provides same day mental health consults, psychosocial assessments, crisis intervention, psychotherapy, alcohol and drug screening using the SBIRT (Screening, Brief Intervention, and Referral to Treatment), education/support groups and resource information. The Behavioral Health Specialist is the link between Camino Health Center, the community, and other local agencies that provide services for this population. Behavioral health services will be provided on site at Camino Health Center and with other community partnerships.

Minimum Position Qualifications:

  • Education: Master’s degree in Social Work from an accredited college, university or professional school
  • License / Certification: Must be licensed, in the state of California, as an LCSW and be in good standing with the Board of Behavioral Sciences; current BLS certification
  • Experience / Training: 3 years of providing direct behavioral health services to patient of all ages; ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions; should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong communication skills

Preferred Position Qualifications:

  • Experience / Training: Bilingual English/Spanish; experience in an out-patient or community mental health setting; previous experience with motivational interviewing and brief counseling
