Job Summary:

Under the general supervision of the Site Director, the Nursing Manager is accountable for planning, organizing, coordinating and managing the operations of the Care Coordination Department and Laboratory. Assures that quality care is provided in the most effective and efficient manner, coordinating service delivery and relationships with community resources and community partners. Adheres to state and federal guidelines relating to primary care and laboratory services. Formulates, recommends, evaluates and interprets clinical policy in accordance with standards. Works collaboratively with the Chief Medical Officer and Quality Improvement Manager to support the efforts to ensure delivery of effective patient care services while promoting a safe environment. Provides supervision to the Care Coordinators and Lab Technician.

The Nursing Manager is an active member of the Management Team.

Job Summary:

Direct Patient Care: Provide high quality, patient-centered and values-based medical care, referral of patients for specialty care as needed, collaboration with health care team members as appropriate for consultation, referrals and follow-up care (80%).

Supervision of Clinical Staff: Assist mid-level health care providers in assessment and treatment of patients as necessary and direct supervision of nursing staff (10%).

Patient Education: Direct patient education regarding medical conditions and treatment, appropriate verbal and written patient referrals for further health education, assist CEO and CMO in program development and implementation as necessary (5%).

Quality Management and Utilization Review: Practice in an efficient, cost effective manner, participation in department and clinic-wide meetings, attend relevant Continuing Medical Education courses (5%).

Job Summary:

The medical assistant’s primary job function is to work as part of the primary care team to provide high quality, efficient and service oriented patient care while demonstrating the health center’s core values. The medical assistant works alongside physicians, mid-level providers, behavioral health professionals, registered nurses and other support staff support to delivery direct patient care services. The medical assistant is responsible for executing standing orders as well as anticipating the needs of the care team in order to help facilitate the delivery of quality clinical care and a positive patient experience. The medical assistant is responsible for actively participating in the health center’s quality and continuous improvement activities.

Job Summary:

The Human Resources Coordinator will report directly to the Human Resources Manager and will be responsible for assisting in the day-to-day human resources operations. The HR Coordinator will provide administrative support for HR functions, as directed, but not limited to on-boarding, recruitment/employment, training, compensation, benefits, leaves of absence, and staff events. The HR Coordinator will be responsible for coordinating and managing the credentialing and privileging process. The Human Resources Coordinator will provide frontline customer service for the organization, including external customers.

Job Summary:

Under the Chief Integration Officer’s supervision, the Maintenance – Handyman is responsible for assisting and coordinating facility operations. In collaboration with the Chief Integration Officer is responsible for coordinating safety activities to support a safe work environment. The Maintenance – Handyman is responsible for implementing a preventative maintenance program that includes keeping and repairing the health center’s facilities and equipment in high performing manner. This individual will coordinate the security and safety for all patients and employees.

  • Education: High School Diploma or equivalent from an accredited school.
  • Experience: Broad knowledge and experience in general repairs, building security, and safety. Experience managing vendor agreements and contracts.

Preferred Position Qualifications:

  • Education: Bachelor’s degree in relevant field from an accredited university.
  • Experience: Two (2) years of experience working in a healthcare setting. Prior experience leading clinic facility operations in a Federally Qualified Health Center (FQHC). Bilingual (English/Spanish).

Job Summary:

Under the supervision of the Site Director, the Clinic Manager is responsible for coordinating the efficient flow of medical, behavioral health and dental services, ensuring a high level of customer service and managing the health center’s telecommunications and electronic health records systems. The Clinic Manager is responsible for organizing and coordinating the day-to-day activities of the medical and dental support staff. The Clinic Manager is charged with maximizing appointment scheduling, ensuring patients are check-in accurately, and facilitating proper registration for public and privately funded programs. Participates and leads process improvement initiatives to enhance the health center’s Patient Centered Medical Home (PCMH) care delivery model. The Clinic Manager is responsible for promoting positive working relationships with patients, families, health care providers and health center employees in the spirit of Camino’s core values. The Clinic Manager is charged with developing a culture of teamwork within the health center in order to ensure an excellent patient experience, efficient operations and optimal access to care.

The Clinic Manager directly supervises the Health Information Clerks, Patient Services Representatives, Patient Services Representative Leads, Medical Assistants, Primary Care Team Leads, Perinatal Coordinator, Dental Assistants, Registered Dental Assistants, and Dental Leads.

The Clinic Manager is an active member of the Management Team.

  • Experience / Training: Prior supervisory experience and excellent customer services skills. Relevant work experience will be considered in lieu of formal education.

Preferred Position Qualifications:

  • Education: Master’s degree in related field
  • Experience / Training: Bilingual English/Spanish; two years of experience working in an outpatient primary care clinic; two years of supervisory experience; experience working at a Federally Qualified Health Center (FQHC).

Job Summary:

The Behavioral Health Specialist provides behavioral health services to patients of all ages at Camino Health Center. The Behavioral Health Specialist work alongside the health center’s medical teams to provide comprehensive whole person care. The Behavioral Health Specialists provides same day mental health consults, psychosocial assessments, crisis intervention, psychotherapy, alcohol and drug screening using the SBIRT (Screening, Brief Intervention, and Referral to Treatment), education/support groups and resource information. The Behavioral Health Specialist is the link between Camino Health Center, the community, and other local agencies that provide services for this population. Behavioral health services will be provided on site at Camino Health Center and with other community partnerships.

Minimum Position Qualifications:

  • Education: Master’s degree in Social Work from an accredited college, university or professional school
  • License / Certification: Must be licensed, in the state of California, as an LCSW and be in good standing with the Board of Behavioral Sciences; current BLS certification
  • Experience / Training: 3 years of providing direct behavioral health services to patient of all ages; ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions; should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong communication skills

Preferred Position Qualifications:

  • Experience / Training: Bilingual English/Spanish; experience in an out-patient or community mental health setting; previous experience with motivational interviewing and brief counseling
