CHC Document Checklist

One Document Per List

Proof Identity (front & back):

  • Identification Card
  • Driver’s License
  • Passport
  • Birth Certificate

Proof of Residence:

  • Utility Bill
  • Rent or Mortgage Agreement

Proof of Income

  • Pay Stub or Previous Year Tax Information
  • Bank Statement
  • Letter from your employer
  • Written Letter Declaring Income (Signed & Dated)

Social Security:

  • If applicable, but not required
